M.S. Degree (Thesis Option)

M.S. (Thesis) Plan of Study Form
This form is used to chart the coursework component of the degree. It should be submitted to the Director of Graduate Studies at the beginning of the semester of graduation.

Master’s Thesis Approval Form
This form is used to request the formation of the thesis committee. It should be submitted to Mr. Jamie Hayes, usually by the start of the semester in which the thesis is to be defended. This form requires the signature of the advisor and the consent of all committee members.

M.S. Thesis Form
This form certifies the successful defense of the thesis. It is usually signed at the time of the defense. The original signed form is to be submitted to the College of Arts and Sciences Office of Graduate Services, with a copy to Mr. Jamie Hayes.

M.S. Degree (Project Option)

M.S. (Project) Plan of Study Form
This form is used to chart the coursework component of the degree. It should be submitted to the Director of Graduate Studies at the beginning of the semester of graduation.

M.S. Project Registration Form
All MS students electing the project option must complete the attached form before registering with your project advisor. It should be submitted to the Director of Graduate Studies at the beginning of the semester of graduation.

M.S. Project Approval Form
This form certifies the successful defense of the project. It is usually signed at the time of the project presentation. The original signed form is to be submitted to the College of Arts and Sciences Office of Graduate Services, with a copy to Mr. Jamie Hayes.

Advisor Meeting Form
This form is used to certify meetings with potential advisors and should be submitted to the Associate Director of Graduate Studies.

MS Advisor Change Request Form
This form is used to officially request an advisor change. This form should be submitted to the Associate Director of Graduate Studies.

Ph.D. Degree

Ph.D. Plan of Study Form
This form is used to chart the coursework component of the degree. It should be submitted to the Director of Graduate Studies at the beginning of the semester of graduation or in the semester in which the student takes their last course.

Old Ph.D. Plan of Study Form (applicable only to students admitted in Fall 2012 or prior semesters)
This form is used to chart the coursework component of the degree. It should be submitted to the Director of Graduate Studies at the beginning of the semester of graduation or in the semester in which the student takes their last course.

Ph.D. Advisor Request Form
This form is used to officially request an advisor. It must be completed within two weeks of passing the research examination. This form should be submitted to the Director of Graduate Studies.

Ph.D. Advisor Change Request Form
This form is used to officially request an advisor change. This form should be submitted to the Associate Director of Graduate Studies.

Ph.D. Committee Request Form
This form is used to request the formation of the dissertation committee. It should be submitted to Mr. Jamie Hayes, usually by the start of the semester in which the dissertation proposal is to be defended. This form requires the signature of the advisor and the consent of all committee members.

Ph.D. Proposal Form
This form certifies the successful defense of the dissertation proposal. It is usually signed at the time of the proposal defense. This form should be submitted to Mr. Jamie Hayes.

Ph.D. Dissertation Form
This form certifies the successful defense of the dissertation. It is usually signed at the time of the dissertation defense. The original signed form is to be submitted to the College of Arts and Sciences Office of Graduate Services, with a copy to Mr. Jamie Hayes.
